Can anyone help me understand all the versions:

Rev 1.15
FW 1.29
Volumio 1.99

It's a little bit confusing, I guessing FW is the XMOS DSP firmware, Volumio software is 1.99, what is Rev, is there any other firmware, is Volumio actually "Volumio OS", if so, why is it version 1.99 on the SHD when it's 3.235 over at Volumio HQ, if it isn't Volumio OS then do we update the Linux version on the ARM board separately?

Huh, this is just confusing as, so they upgrade the firmware but don't change the version???

Rev 1.15 / FW 1.28 / 8 March 2021 (minor) - Bug fix IR repeated previous command

Rev 1.15 / FW 1.29 / 16 March 2021 (minor) - Volume control update for new Volumio build 1.99. This update is required when updating to Volumio 1.99 as it modifies volume control format for Roon compliance (pending final certification).

Rev 1.15 / FW 1.29 / 16 March 2022 (minor) - Recompiling OSx build to fix Monterey file access bug. No update required if you're not on Monterey.

Rev 1.15 / FW 1.29 / 21 March 2022 (minor) - Low SNR bug on OSx. Related to Microphone access. No update required if you're on Windows. 

SHD Software for Dirac 3.x Published on 21 March 2022  Modified on 24 March 2022 (SHD_v1_15_fw_v1_29.zip) - Software for the SHD series with Dirac 3.x support. Includes plugin and firmware. Dirac 3.x software must be separately downloaded from Dirac website (live.dirac.com/). To get started, make sure to read our SHD series user manual and Dirac live 3.x User manual.

There are three different things that can be updated: the SHD firmware, the Volumio version installed and the SHD plugin installed on the computer used to configure the SHD. The different version numbers you're seeing refer to each of these. I'm sure this is covered in the manual if you have a look.
